This subscription agreement (the “Agreement”) is between you (“Patient”) and Arnold Chiropractic LLC (“Provider”) and is effective upon the date of enrollment.

1. Subscription Overview
By subscribing, the Patient gains access to the following services each month for four (4) months:
Two (2) Wellness Room Sessions: Each session includes access to vibroacoustic and low-level light therapy. Sessions are 30 minutes in length.
Two (2) Chiropractic Adjustments: Chiropractic services must be scheduled separately but may be scheduled in back to back time slots and are subject to availability.
The Patient is responsible for scheduling their services each month. Unused sessions or adjustments cannot be carried over to subsequent months.

2. Subscription Details
Four-Month Term: The subscription is for a fixed term of four (4) consecutive months.
Promotional Rate: The total cost is $89 per month during this promotional period.
Non-Renewing Subscription: The subscription will automatically end at the conclusion of the four-month period.

3. Payment Terms
Payment Authorization: By enrolling, the Patient consents to automatic charges to the credit card on file via Stripe.
Billing Cycle: Payments will be processed on the same date each month, based on the subscription start date.
Failed Payments: If a payment fails, the Patient must update their payment method within five (5) business days. Services will be paused until the issue is resolved.

4. Scheduling and Appointment Policies
Wellness Room and Chiropractic Appointments: The Patient is entitled to two (2) 30-minute Wellness Room sessions and two (2) chiropractic adjustments per month.

Cancellations and Rescheduling:
Patients must provide at least 48 business hours’ notice to cancel or reschedule appointments.
Missed appointments or cancellations with less than 48 hours’ notice will count toward the monthly allowance.
Appointments may be canceled or rescheduled via phone, email, or the patient portal.
Transfer of Sessions: With prior authorization, unused sessions may be transferred to a dependent child or spouse of the Patient.

If the Provider needs to cancel a session due to unforeseen circumstances, the patient will be notified as soon as possible and the session will be rescheduled for the next available appointment that works for both the patient and the provider.

5. Subscription Pausing and Non-Use
No Pauses During Promotional Period: This fixed-term subscription does not allow for pauses or extensions.
Unused Services: Unused sessions or adjustments do not carry over to subsequent months.

6. Health, Safety, and Guidelines
Health Disclosure: The Patient affirms they have disclosed any relevant health conditions to the Provider and understands the nature of the services offered.
Safe Use: The Patient agrees to follow all instructions for safe use of the Wellness Room. If discomfort occurs, the Patient must stop immediately and notify the Provider.

7. Wellness Service Understanding
Wellness Room services and chiropractic adjustments are designed to promote relaxation and general wellness. They are not intended to replace medical treatment or advice.
The Provider does not guarantee specific outcomes, and participation is voluntary.
These statements have not been evaluated by the FDA. Services are not intended to diagnose, treat, cure, or prevent any disease.

8. Subscription Termination
End of Promotional Period: The subscription will automatically terminate at the end of the four-month term, with no further action required by the Patient.

Termination by Provider: The Provider reserves the right to terminate this agreement at any time if the Patient violates the terms or engages in unsafe or damaging behavior.

Agreement to Terms
By enrolling in this subscription, the Patient confirms they have read, understood, and agree to these Terms and Conditions, including the authorization for recurring payments.
